    Half life of chloroquine

In addition, the impact of the patient’s physiopathological status and ethnic origin on chloroquine pharmacokinetics is discussed. In humans, chloroquine concentrations decline multiexponentially. The drug is extensively distributed, with a volume of distribution of 200 to 800 L/kg when calculated from plasma concentrations and 200 L/kg when estimated from whole blood data (concentrations being 5 to 10 times higher).

In contrast to quinine which has a relatively limited apparent volume of distribution approximately 2.51/kg in healthy adults and short elimination half-life 11 hours in healthy subjects, 16 hours in cerebral malaria, chloroquine has an enormous total apparent volume of distribution 100–10001/kg and a terminal elimination half-life of approximately one month White et al. 1985. The pharmacokinetic properties of chloroquine are complex; distribution rather than elimination processes. Half-life time of this drug is approximately 18 hours in vivo due to the degradation by the liver, but the stability of chloroquine in vitro experiments is expected to be longer. The plasma half-life of chloroquine in healthy individuals is generally reported to be 72-120 hr. In one study, serum concentrations of chloroquine appeared to decline in a biphasic manner and the serum half-life of the terminal phase increased with higher dosage of the drug.

Following administration chloroquine is rapidly dealkylated via cytochrome P450 enzymes (CYP) into the pharmacologically active desethylchloroquine and bisdesethylchloroquine. Chloroquine is 60% bound to plasma proteins and equally cleared by the kidney and liver.

A metabolite of chloroquine – hydroxycloroquine – has a long half-life of 32–56 days, so it's not a good idea to use a single dosage past one month anyway. Using CP in a display tank or any tank with rock/substrate almost always fails to eradicate the parasites. One study indicated that chloroquine declines in a biphasic manner, with the terminal phase half-life increasing with increasing doses, as follows 3.1 hours after a single 250-mg oral dose, 42.9 hours after a single 500-mg oral dose, and 312 hours after a single 1-g oral dose. Chloroquine overdose is a life-threatening emergency and should be managed with cardio-respiratory and hemodynamic support, monitoring of potassium along with management of arrhythmias and convulsions, as necessary.
