To characterize structural changes of HCQ retinopathy with SD-OCT after drug cessation. Setting: Private practice and academic institution. Patient Population: Patients at New England Eye Center and Ophthalmic Consultants of Boston in Boston, MA diagnosed with HCQ retinopathy and followed after drug cessation.

Chloroquine is an inhibitor of the lysosomal degradation of the DNA which is taken up by the cells, so as leelee said, transfection should have been successful, albeit at a slightly lower level than if you would have added the chloroquine. In my normal transfections, I never add chloroquine and get high efficiencies nevertheless. Aug 06, 2014 Previously described OCT findings in HCQ toxicity include loss of the external limiting membrane, disruption of the outer ellipsoid zone, parafoveal thinning of the outer nuclear layer and RPE damage. 6,7,10 Despite these various changes, numerous studies have supported the notion that relative “foveal resistance” is common in HCQ toxicity, as demonstrated by preservation of the subfoveal outer retinal layers, accounting for the intact central visual acuity that can be seen even in. In obvious toxicity, the nasal inner subfield showed more thinning than the temporal inner subfield at 12 months after drug cessation p = 0.018, 95 % CI −1 to −8 μm. Conclusions Once HCQ retinopathy is diagnosed and the medication is discontinued, structural retinal changes commonly occur.

Main Outcome Measures: SD-OCT findings suggestive of HCQ retinopathy before parafoveal ellipsoid disruption. Retrospective clinical data review by the Boston Image Reading Center.

  6. The estimated risk of hydroxychloroquine retinopathy in patients on chronic hydroxychloroquine therapy ranges from 0.4% to 8.0%. 1 Toxicity is thought to be dependent on cumulative total dose. 3 Additional factors that can precipitate early toxicity are concomitant renal disease and use of tamoxifen. 3 Figure 1.

SD-OCT is an objective test for toxicity that complements 10-2 visual field testing. 10-2 visual testing is probably more sensitive, but is heir to interpretational difficulties and variability not so much a problem with SD-OCT. Hydroxychloroquine Plaquenil; HCQ has been an important and effective drug for the treatment of lupus erythematosus and related autoimmune and inflammatory diseases for half a century, although its potential to cause retinal damage continues to raise concern among rheumatologists and ophthalmologists. Allergic to Plaquenil, new to Prednisone lupus I am allergic to plaquenil too. I went through about a 6-8 month period of trying different combinations of meds to figure out what works for me to keep the Lupus under control. I personally don't like to take steroids unless I absolutely have to, to control a flare.
