An incomplete annular scotoma in the zone from 2 to 8 degrees from fixation developed in the third VF. This scotoma was reproducible and enlarged from 2011 to 2012. Visual Fields Early toxicity can cause bilateral relative paracentral scotomas. 4 Defects can be present BEFORE definitive signs are seen on fundus examination. Some experts suggest using a red stimulus or blue-yellow perimetry for early detection of retinopathy. 5 Figure 3 Five consecutive 10-2 visual fields VFs with a white target. 5. Abstract. Background The American Academy of Ophthalmology recommendations on screening for chloroquine CQ and hydroxychloroquine HCQ retinopathy are revised in light of new information about the prevalence of toxicity, risk factors, fundus distribution, and effectiveness of screening tools.

How do I run visual fields? Visual field instruments are kept in dim spaces, and testing is run in dim illumination. Bright light exposure for too long can throw off the calibration of the field, so avoid it if you can. Bowl-based perimetries require the patient to be patched. Paracentral scotomas have been reported prior to the associated maculopathy, so an amsler grid test or central 10-2 visual field perimetry was recommended. 2,5 Additionally, color vision––helping in the assessment of macular integrity––was recommended as an optional test. Although visual field defects in HCQ toxicity can be highly variable in terms of the depth of defect, all our patients showed a similar pattern on 10-2 white, with the same areas affected. However, one needs to look carefully for subtle decreases in threshold in the area at risk 2–6 degrees from center.
